Further refinement of reading restart data from Exodus throug stk_io and Ioss.

Added extra argument to "populate_bulk_data()" for time index
to read from. Use this index for begin_state() and end_state()
call on the region.

In Ioex_Database, put in an assert(step>0) to fix problem of
reading at an uninitialized state value.
